Question:
Does the ISO 9001:2008 standard require pages to be numbered in a controlled document? Example: in an SOP would you have to put page 2 of 3? Or as long as the bottom has issue date, form #, revision number and revision date – is that ok?

Answer:
It is not necessary to put Page x of y on the document, but there should be a way of knowing how many pages there are in a document, or which is the last page of the document. Using Page x of y is the best way to do this. Another approach is to identify the last page as “last page” or “end”. Otherwise it will be difficult to know if you have all the pages to a document, especially when it is in hard copy. Everyone in the organization should understand the approach that is used.
